Community Based Services

Miracle Works’ Community Based Services empower individuals/families to successfully participate in a community environment by providing services that are responsive, respectful, and adaptive to individual needs and preferences. Services are strengths-based and flexible to meet the family needs.
Supervised Visitation
Provides a positive and structured atmosphere where parents and children may interact in a supervised and safe environment, while protecting the integrity of family visits.
Behavior Management
Provides services to develop and implement a strengths-based, individualized behavior plan that is designed to meet a specific Service Coordination Plan outcome.
Educational Mentoring
Provides a developed and structured one-to-one relationship with a client for the purpose of addressing identified needs in an educational setting.
Recreational/Social Mentoring
Provides a consistent and structured one-to-one relationship with a client for the purpose of addressing emotional or behavioral needs in a recreational or social setting.
Tutoring
Provides tutoring by Miracle Works Staff in academic areas or organizational skills for the purpose of addressing identified educational needs. Providers will possess expertise in the specific subject or skill area.
Life Coach/Independent Living Skills Mentor
Provides a developed and structured one-to-one relationship with a client for the purpose of addressing daily living needs.
Individual Therapy
Provides outcome based therapeutic intervention with a child or family member. Services will be structured, goal-oriented, time-limited therapy in the natural environment of families who need assistance recovering from the following: physical, sexual, emotional abuse, and neglect. Additionally, the therapists will provide support to families related to abuse and neglect, such as: substance abuse, mental illness, personality/behavior disorder, developmental disability, dysfunctional family of current family dysfunction.
Case Management
Provides outcome focused, needs based activities which assist families and children. Services will be high quality, family centered and culturally competent. Miracle Works staff will focus on reducing maltreatment, improving caretaking and coping skills, enhancing family resilience, supporting healthy and nurturing relationships, and children’s physical, mental, emotional and educational well-being.
Family Preservation
The service shall include assessment of child/parent/family resulting in an appropriate service/treatment plan that is based on the assessed need. Participation in Child and Family Team Meetings to assist with continuous development of family goals.
